How to access 3L Care Services

Residential, Transitional Respite Stays, Day Care

Knowing how to access any care plan can be daunting and you should know that you don’t need to do this alone! We are here to help you make the process as straightforward and stress-free as possible.

The process is carried out by following these simple steps:

Discuss the service that you are interested in accessing with Michelle.

Arrange to come and have a look around our homes.

R

If you like what you see and you would like to arrange a place with 3L then we will arrange to have an assessment of need carried out.

R

You will inform your social worker that you require their support and pass on our details to them.

R

Our nursing team will carry out an assessment of need with you and your dependent and will prepare a report and a fee schedule.

R

The pricing schedule is sent to the family, commissioner and social worker for review.

R

Self-funding individuals can proceed to access the services directly.

R

Funding applications tend to go to panel with the commissioning body and together we ensure the smooth running of the submission by ensuring that all of the relevant points are covered.

Once the panel agree the funding, 3L Care provides a commissioning document which is signed by the family/individuals’ representative, the commissioners and 3L. It details the financial arrangements to ensure that the provision of care is not affected by any financial restrictions or delays in payments.

An introductory visit is then arranged for you and your dependent.

R

A ‘Moving In’ day is agreed and we prepare for the arrival of your dependent, you and/or their ‘settling in team’.

Transitional Respite Stays can be booked in advance as part of the transitional plan to residency.

R

Day care services can also be booked 12 months in advance.
